Ishak agé de 5 and est atteint d'un cancer du rein et a eu aussi un AVC qui rend son etat de sante critique.

Il doit subir un long protocole de traitement chimiotherapie, intervention chirurgicale et radiotherapie (proton) qui ne se fait qu'en France.

le cout estimatif de son traitement s'eleve a plus de 200000 Euro

2-Malignant thrombus in the IVC up to the level of right atrium 3- Metastasis to the right iliac bone /left 8th rib
4- Biopsy taken on 16.05.2023 (result pending)

Date last seen: Inpatient Active issues: - Hypertension under hypertensive medication
Current Medication: -Hydralazine 3mg IV as needed
Clinical Progress:
Ishak waswell till 11th ofMay when he started to have repeated vomiting, abdominal pain and dark urine. Family took the child to a private hospital where US abdomen showed right renal mass.
CT scan of the chest and abdomen also were done on 12.05.2023, the CT scan of the abdomen showed large heterogenous hypodense soft tissue mass occupying the right kidney,totallyencasing the right renal artery, it is seen infiltrating the pelvi-calyceal system and right upper ureter. Right renal vein infiltration and partially filling the IVC measuring about 10 cm just before the right atrium.
CT scan of the chest showed erosion of the left 8th rib with underlying soft tissue density.
Upon this result patient was referred the pediatric surgical side where biopsy and port-A-cath was inserted on 16.05.2023, histopathology still pending then he wasreferred again to the pediatric oncology unit on 17.05.2023 forfurthermanagement. On 19.05.2023 Echo-cardiography was done and EF was 76%, patient is under regular monitoring of blood pressure.
He wil need chemotherapy according to final histopathologyreport, with surgical resection ofthe mass. Thrombectomy with extracorporeal circulation and finally radiation therapy (preferably proton therapy)
